The Nutrition Care Plan is developed by a nutrition professional based on the assessment, diagnosis, and intervention stages of the Nutrition Care Process. The Nutrition Care Process is a systematic approach used by nutrition professionals, as well as the CHE -DFSN Nutrition clinic to provide individualized care and improve the health and well-being of individuals. It involves a series of steps to assess, diagnose, intervene, monitor, and evaluate a person's nutritional status and needs.

NUTRITION ASSESSMENT
This is the first step where the nutritionist dietitian collects information about the individual's medical history, lifestyle, dietary habits, and any specific concerns or goals they may have. This helps to understand their current nutritional status and needs.

DIAGNOSIS
Based on the assessment, the nutritionist dietitian identifies any nutritional problems or diagnoses, such as malnutrition, food allergies, or weight management issues. This step helps in determining the specific areas that need attention and improvement.

INTERVENTION
In this step, the nutritionist dietitian develops a personalized plan of action to address the identified nutritional problems. This may involve setting dietary goals, recommending specific foods or nutrients, providing meal plans, and suggesting lifestyle modifications. The interventions are tailored to meet the individual's unique needs and preferences.

MONITORING
Once the intervention plan is in place, it's essential to monitor the individual's progress regularly. This involves tracking their dietary intake, body measurements, and any relevant health parameters. By closely monitoring the changes, the nutrition professional can make adjustments to the plan as needed and provide ongoing support.

The final step involves evaluating the effectiveness of the intervention and assessing the outcomes achieved. The nutrition professional compares the individual's progress against the initial goals and measures the impact of the intervention on their nutritional status and overall health. If necessary, modifications are made to further optimize the plan.
The CHE – DFSN clinic offers hybrid or blended learning of nutritional training programs and resources designed to enhance knowledge and understanding of nutrition principles, dietary guidelines, and healthy eating habits.
The CHE – DFSN Nutrition clinic are affiliated with different health institutions focusing on the needs of the at-risk groups which enable students to apply learned theories into practice. The experience will enable the students to interrelate nutrition, growth, development, and maintenance of health at different stages of the life cycle of at-risk groups with emphasis on mothers, infants, children, adolescents, and older adults.
The CHE – DFSN Nutrition Clinic is in partnership with selected specialized hospitals which offer additional hospital dietetics training. The training aims to equip graduate students with the knowledge, skills, and tools to become competent nutrition professionals who can provide evidence-based nutrition advice and education to individuals and groups with disease/conditions using appropriate nutrition care plans and prepare them for careers in clinical nutrition-related fields. This is designed for licensed nutrition graduates.
The CHE – DFSN Nutrition clinic is open to thesis and project-based partnerships and collaborations across sub-disciplines of Home Economics, among UP units, NGOs, different healthcare institutions, and industry.
